ACKNOWLEDGEMENT AND ACCEPTANCE OF RISK

Activity’s inherent risks

I acknowledge having been informed about the risks of surfing, which are part of Barefoot’s activities. The risks are, more specifically, but not limited to, personal injury, trauma or drowning. I agree in full knowledge of the nature of these risks as part of the activity, and the consequences that may result.

I also acknowledge that, given the distance and location, evacuation may be long and arduous and that medical care won’t be available in a short amount of time. I understand that staff members may exclude any person from the group that may represent a hazard to others.

Health status

Do you have any physical, emotional or behavioural problems that would limit you directly or indirectly in the practice of the activity in which you participate?

If so, you must meet with a staff member and tell him.
Permission to give first aid
In case of injury, accident or lost of consciousness, I authorize Barefoot’s staff to provide me the necessary emergency care and take appropriate action to carry me to the hospital.

If I take medication or if I know I can have special reactions to certain situations, I notified the staff. I have shown what to do in the case of inability to give myself the treatment.

CONFIRMATION OF INFORMATION AND ACCEPTATION OF RISK

I certify that the information contained here is accurate, and that to the best of my knowledge. I certify that I have not deliberately withheld information about my health. I am aware that the information contained in this form is confidential and is intended to better plan and supervise the safety of activities in which I will attend. I am aware that the activities offered by Barefoot take place in natural terrain, that could cause injury and that are far from any medical service. This fact could cause long delays in an emergency requiring an evacuation, and therefore a possible worsening of my condition or my injury. Having considered these risks and having had the opportunity to discuss with a person responsible for the activity, I acknowledge having been informed of the inherent risks and I am able to undertake the activity or trip KNOWING AND ACCEPTING THE RISKS that may occur during this trip or activity. I also promise to play an active role in managing these risks by adopting a preventive attitude towards me and in respect of other people around me. I am responsible for the equipment that I have borrowed and I will pay the damage fees if I do damage on it. I will not recognize Barefoot as responsible for any of my personal goods being stolen. I understand that I may leave the present activity for this reason or for another.
